National Disability Employment Awareness Month: Voluntary Self-Identification of Disability Form



In concluding the October National Disability Employment Awareness Campaign, Human Resources posted a letter with links to the Voluntary Self-Identification of Disability Form. This launched the invitation to active employees to complete this form and return it back to Human Resources by November 30, 2015.

One such employee reached out and shared that they had already self-identified as a Veteran, but through the awareness of the October 2015 campaign, realized they had never identified as a person with a disability and felt they would now like to self-identify. 

Human Resources would like to encourage all employees to do the same. Please complete the voluntary self-identification form that was sent to your HPU email, and return it to Human Resources at hr@hpu.edu or via inter-office mail, or simply drop it off at the HR Office Downtown in UB on the 8th floor.
