National Disability Employment Awareness Month: Office of Human Resources Launches Disability Self-Identification Survey


Hawaiʻi Pacific University strives to achieve equal employment opportunity throughout the organization for all employees. In support of this, the university is launching a Disability Self-Identification Survey for all employees. The online form can be found on HPU Pipeline and hard copy forms can be requested from Human Resources by contacting Debbie Cairme, Employee Relations, Investigations, EEO/AA Compliance Director, at dcairme@hpu.edu. If you have any questions regarding the form, please feel free to call her at (808) 544-1191.

As we celebrate National Disability Employment Awareness Month here at HPU, we are inviting all of our employees to self-identify if they have a disability. The process is confidential and easy to do by completing the Voluntary Self-Identification of Disability form (Form CC-305) found on HPU Pipeline.

This will help identify those with qualified needs and the corresponding reasonable accommodation to meet those needs.

The following video clip, “Disability Inclusion Starts With You”, found on the OFCCP (Office of Federal Contract Compliance Programs) website, highlights the importance of Self-Identification and every employer’s obligation to seek out their employees’ disability needs.
